đź’¬ What Is Social Prescribing?
Social prescribing is a way of connecting you with non-medical support in the community to help improve your overall wellbeing. Many of the issues people bring to their GP are not medical they’re social, emotional, or practical. That’s where social prescribing comes in.
Through this service, you can be referred to a Social Prescribing Link Worker a dedicated team member who will take time to listen and understand what really matters to you.
🤝 How Can It Help Me?
You might benefit from social prescribing if you’re dealing with:
- Feeling lonely or isolated
- Mild to moderate mental health issues like low mood or anxiety
- Housing, money or employment challenges
- Managing a long-term condition
- Wanting to be more active or connect with others
- Looking to take part in arts, gardening, music, or volunteering
đź“Ś Why You Might Not Need to See a GP
Sometimes, what you really need isn’t a prescription it’s connection, support, and guidance. If you’re coming to see us about something that doesn’t need medical treatment, like stress from debt, loneliness, or lack of routine, our social prescribing team may be the best place to start.
Our Link Worker can support you to:
- Access local groups and services
- Get practical help with life challenges
- Build confidence and set personal goals
- Improve your health and wellbeing without medication
👩‍⚕️ You’re Still in Safe Hands
Social prescribing isn’t a replacement for your GP, it’s an extension of your care. Your GP or nurse might suggest a referral, or you can ask us about it yourself.
This is a free service, available to all our registered patients.
